Targeted population health assistance for Black and African Americans is an important part of creating a healthy, equitable society. As the largest minority population in the United States, Black and African Americans have some of the most challenging health outcomes of any population. Although advances in medical technology and improved access to care have helped to reduce health disparities, significant gaps remain.
Efforts to close these gaps and improve the overall health of the Black and African American population can take many forms. A first step in bridging these gaps is Crescent's access to high-quality, affordable primary and preventative care. Additionally, we offer targeted health education through classes and one-on-one programs and outreach initiatives to help improve health literacy and encourage healthy lifestyle choices.
At Crescent, we have a dedicated community health worker whose role is working directly with the Black and African American communities to ensure we are removing barriers to improve health outcomes and reduce health disparities for this population.
