Crescent Community Health Center opened its doors in October of 2006. Founded by a group of dedicated board members, made up of the community who identified the need for primary medical and dental care for underserved populations. Since then, Crescent has been providing primary and preventative care to the members of our community.

2006 — A Vision Takes Shape
Crescent Community Health Center is established
Crescent was founded with a clear mission: to provide high-quality, respectful, affordable healthcare for all.
From the very beginning, the focus was on removing barriers and ensuring access to care—regardless of income, insurance, or background.
2007 — Becoming a Federally Qualified Health Center (FQHC)
A major milestone in access and impact
Crescent earns designation as a Federally Qualified Health Center (FQHC)—a significant step that strengthens its ability to serve the community.
Why this matters:
- Expands access to care for underserved populations
- Enables a sliding fee scale based on income
- Provides access to federal funding to support services
- Ensures high standards of quality and accountability
This designation allows Crescent to grow sustainably while staying true to its mission.
2012 — Investing in Prevention
Opening of the Wellness Center
Crescent opens its Wellness Center at 1805 Central Avenue—Dubuque’s historic Engine House #1, creating a free resource for patients focused on prevention and whole-person health.
The space offers:
- Fitness equipment
- Wellness classes
- One-on-one coaching
This milestone reflects Crescent’s commitment to helping patients stay healthy—not just treat illness.
2013 — A Commitment to Quality
Patient-Centered Medical Home Recognition
Crescent receives Patient-Centered Medical Home (PCMH) recognition from the National Committee for Quality Assurance (NCQA).
This recognition reflects a care model that:
- Puts patients at the center of every decision
- Emphasizes coordinated, team-based care
- Focuses on quality, safety, and improved outcomes
It reinforces Crescent’s commitment to delivering care that is not only accessible—but exceptional.
2016 - A Decade of Impact
This moment reflects not just growth—but trust. More individuals and families are turning to Crescent as their healthcare home.
2019 — A New Home for Growing Care
Crescent relocates to 1690 Elm Street
In November 2019, Crescent Community Health Center moved into its newly renovated home at 1690 Elm Street, nearly doubling its space to better serve a rapidly growing patient community.
The new facility brought medical, dental, wellness, and support services together under one roof—creating a more connected, patient-centered experience. Features of the expanded space included:
- Brand new medical and dental clinics
- Expanded exam rooms and dental suites
- An on-site wellness center and training kitchen
- Brain health expansion space
- Patient kiosks and staff training areas
The move marked a major step forward in Crescent’s mission to provide comprehensive, accessible healthcare while remaining rooted in the neighborhood it has long served.
2020 — Milestone: Crescent sees more than 20,000 clinic visits
Expanding Whole-Person Care
Opening of the Brain Health Clinic
Crescent expands its services with the opening of the Brain Health Clinic, recognizing that mental health is essential to overall health.
This addition brings behavioral health services into the primary care setting—making it easier for patients to access support for conditions like anxiety, depression, and other mental health needs.
By integrating brain health into everyday care, Crescent continues its commitment to treating the whole person—mind and body—while reducing barriers and stigma around seeking help.
2021 — Increasing Access to Care
QuickCare Opens
Crescent expands access with the opening of QuickCare on the first floor, offering same-day care for non-emergency needs.
This service provides patients with a convenient option for immediate care—helping reduce wait times and ensuring individuals can be seen when they need it most.
QuickCare strengthens Crescent’s commitment to accessible, responsive healthcare for the community.
2022 — Building for the Future
Expansion, Partnerships, and New Services
Crescent takes major steps to expand access, strengthen partnerships, and invest in the future of care.
Construction begins on a new Outbuilding, creating space for an expanded Wellness Center and a state-of-the-art training facility. At the same time, renovations on the main building’s first floor support an expanded dental hygiene program and improved staff spaces.
Crescent also deepens its commitment to the community through new partnerships:
- A Memorandum of Understanding (MOU) with Dubuque Community Schools to support on-site clinics
- A collaboration with Northeast Iowa Community College (NICC) to grow the dental hygiene workforce
In December, Crescent expands access to affordable medications by purchasing the assets of Infocus Pharmacy, strengthening integrated care for patients.
2024 — Milestone: Crescent serves more than 10,000 patients annually
2025 — A New Era of Growth
New Services and Community Growth
Crescent enters a new era of growth—expanding services and increasing access for the entire community.
Through a $4.55 million capital campaign, Crescent enhances its facilities and broadens the scope of care, including:
- Oral Surgery
- Optometry
- Dermatology
- Pediatric Telepsych
This year also marks an important milestone as the Wellness Center opens to the public, expanding access beyond patients and creating new opportunities for prevention, fitness, and whole-person health.
These additions strengthen Crescent’s ability to provide comprehensive, integrated care—meeting patients where they are and supporting their health at every stage of life.
