Crescent Community Health Center opened its doors in October of 2006. Founded by a group of dedicated board members, made up of the community who identified the need for primary medical and dental care for underserved populations. Since then, Crescent has been providing primary and preventative care to the members of our community.

2006
- Crescent opens its doors.
2008
- Crescent becomes a Federally Qualified Health Center (HRSA)
2012
- Opened the Wellness Center.
2013
- Received the Patient Centered Medical Home recognition from the National Center of Quality Assurance.
- Received the Iowa Pharmacy Association Award as the Community Champion.
- Adopts electronic medical records.
2016
- Expanded access by adding providers to see additional patients to meet growing demand.
2017
- Signed lease for new building at 1690 Elm Street (former Lamar building).
2019
- Dubuque Pacific Islander Health Project Advisory Council adopted.
- Crescent Community Health Centers moves to completely renovated space at 1690 Elm Street in November. To learn more about the move and new space click here.
- Strategic visioning commences in December with the goals of growth, culture of excellence, and outreach.
2020
- Brain Health Clinic opens for patients in newly renovated space on second floor.
2021
- QuickCare clinic opens in newly renovated space of first floor.
2022
- Outbuilding construction begins to move and expand wellness center and offer state-of-the-art training center.
- First floor south construction began for expanded dental hygiene program and staff lounge.
- Memorandum of Understanding (MOU) with Dubuque schools for on-site clinics.
- MOU with Northeast Iowa Community College (NICC) for a hygiene collaboration program.
- Crescent purchased assets of Infocus Pharmacy in December.
2023
- Dental expansion project begins to include new lab and operatories.
