Crescent Community Health Center Welcomes Dr. Sarah Jacobitz-Kizzier
Crescent Community Health Center is thrilled to announce the addition of Dr. Sarah Jacobitz-Kizzier to our dedicated team of healthcare professionals. Dr. Jacobitz-Kizzier, a committed physician mom, brings nearly a decade of experience in caring for families across the tri-state area. Her extensive expertise spans care for newborns, children, teens, and adults, reflecting her deep commitment to advocating for a healthier community.
Dr. Jacobitz-Kizzier’s approach to healthcare is both thorough and efficient, ensuring patients receive the best possible care. She is equipped to provide a wide range of services, including same-day visits, well-child exams, annual physicals, and various procedures such as colposcopy/LEEP, contraceptive management (including IUDs and Nexplanons), skin biopsies, and joint injections.

Crescent was honored with the Gold Health Center Quality Leader, Access Enhancer, Addressing Social Risk Factors, and Advancing Health Information Technology (HIT) for Quality awards. Additionally, in 2023, the Crescent medical clinic saw 13,677 visits.
