At Crescent Community Health Center Pharmacy, medication assistance is part of our broader commitment to whole-person care.

Through partnerships with charitable medication programs—including support from St. Vincent de Paul and Dispensary of Hope—we help eligible patients access medications at no cost.

These partnerships allow us to expand access to essential medications for patients who may otherwise struggle to afford them. This support is not a standalone program—it is fully integrated into your care, working alongside your provider and care team to ensure your treatment is both effective and accessible.

WHO MAY QUALIFY

You May Qualify for Medication Assistance If You:

Are uninsured
OR
Have commercial insurance without prescription coverage

And:

Have a household income at or below 300% of the Federal Poverty Level

Patients with Medicaid, Medicare, VA, or other government insurance are not eligible
