Crescent Community Health Center has been recognized by the U.S. Department of Health and Human Services' Health Resources and Services Administration (HRSA) with four 2025 Community Health Quality Recognition (CHQR) badges, recognizing the organization's commitment to providing high-quality, accessible, and patient-centered care.
Crescent Community Health Center received recognition in the following categories:
- High-Value Care – Honors health centers that have achieved significant improvements in clinical quality while managing the growth of health care costs.
- Improving Health Care Access – Recognizes health centers that have expanded access to care while demonstrating improvements in quality and increasing the number of patients served.
- Heart Health – Awarded to health centers that meet or exceed national benchmarks for cardiovascular care measures, including blood pressure control, tobacco cessation, appropriate statin therapy, and other evidence-based heart health initiatives.
- Advancing Health Information Technology – Awarded to health centers that effectively use health information technology to improve care, including telehealth services, electronic exchange of clinical information, patient engagement, and collection of patient health-related needs data.

Community Health Quality Recognition badges are awarded annually by HRSA based on the most recent Uniform Data System (UDS) reporting period. The awards recognize health centers across the country for achievements in clinical quality, health outcomes, access to care, and effective use of health information technology.
These awards reflect Crescent Community Health Center's ongoing commitment to expanding services and improving health outcomes for patients throughout the Dubuque region. Over the past year, Crescent has continued to grow access to comprehensive medical, dental, brain health, pharmacy, oral surgery, dermatology, digital optometrics, wellness, and specialty care services.
As Crescent celebrates its 20th anniversary this year, these national recognitions reinforce the organization's mission of providing exceptional care regardless of a patient's ability to pay.
